Transforming community health in Uganda: The journey of the electronic Community Health Information System

Uganda’s transformation to digital healthcare delivery is a story of innovation, collaboration, and perseverance through the challenges.

In rural Uganda, reaching a health facility poses a significant challenge. Community health workers (CHWs) serve as a critical link, bringing life-saving care directly to households, recording births, and tracking immunizations.

For years, they relied on paper-based systems, making it difficult to understand the urgency of health crises or make quick decisions in critical situations.

To address this, Uganda’s Ministry of Health envisioned a transformative solution: an electronic Community Health Information System (eCHIS) that would facilitate real-time data collection, seamless reporting, and timely follow-ups across health services.

From vision to reality: The birth of eCHIS

eCHIS wasn’t just a technical upgrade; it was a bold commitment to strengthening Uganda’s healthcare system and improving millions of lives.

Recognizing the complexity of Uganda’s healthcare system, the Ministry brought together NGOs, technology partners, and CHWs. These discussions unearthed frustrations with paper systems and shaped a system that reflected the needs of the people it serves.

After careful evaluation, Uganda selected the Community Health Toolkit (CHT) for its adaptability, open-source design, and offline capabilities, which make it ideal for rural areas with limited connectivity. The platform was customized to support priority workflows like Integrated Community Case Management (ICCM), Maternal and Child Health (MCH), and household registration.

Piloting change: Bridging challenges with training

Piloting the system in four districts was the real test. There was initial hesitation from CHWs: some were unfamiliar with technology, while others worried about the new system’s complexity. Through hands-on training, peer mentoring, and iterative feedback, their confidence and trust grew. 

Equipped with successful pilots, and multisectoral funding, eCHIS expanded to more districts, reaching over 15,000 CHWs by 2024. Training programs equipped biostatisticians and national trainers to ensure long-term sustainability.

Driving impact: Real-time data and improved care

What makes eCHIS unique is its commitment to collaboration. 

The Ministry’s technical teams worked alongside partners to manage the system, grounding every step of the rollout in national and district expertise. In 2023, eCHIS was translated into the regional language Luganda, making it even more user-friendly for individuals in Buikwe District.

The journey has not been without its challenges. Limited connectivity in rural areas created difficulties in uploading data. The offline functionality of the eCHIS proved invaluable, enabling CHWs to synchronize data once connectivity improved. 

The eCHIS is now revolutionizing community healthcare in Uganda. CHWs are now recording accurate patient data in real time, enabling quicker decision-making and more accurate treatment and referrals. Integration with national systems like DHIS2 means data flows seamlessly from communities to policymakers, guiding effective resource allocation..

In the community, children are being vaccinated on time, and mothers are attending critical antenatal care visits. To date, eCHIS has facilitated assessments for 460,826 children under-five, leading to 76,988 sick children being referred to facilities to receive the critical care they needed.

The road ahead: Expanding possibilities

As eCHIS continues to evolve, the Ministry is focused on the future, fostering partnerships to ensure the eCHIS can be scaled for long-term success.

Plans include integrating eCHIS with the client and CHW registries, streamlining data across all healthcare levels. Advanced analytics and AI will help predict outbreaks, analyze trends, and allocate resources more effectively. User feedback will remain central to ensuring that eCHIS adapts to the needs of CHWs and the communities they serve.

Uganda’s journey with eCHIS is far from complete. As the system continues to develop, its potential to inspire other countries grows, showing how digital systems can pave the way for a healthier and more connected future for everyone.
